Sometimes, knowing where to start may be the hardest part of taking care of your health, changing health habits or making the most of your benefits. The goal of CareConnect Premier is to provide you with access to advocates and benefits specialists to help answer some of your questions or discuss health concerns, when it is not an emergency. Consider calling the phone number at the top of the page with everyday questions such as:

• How do I find a new doctor that is in network?
• How can I help avoid costly and unnecessary trips to the ER?
• How do I treat a minor injury?
• I’m not feeling my best. I wonder which care option I should use?

Or call to get information about a specific health condition such as asthma, back pain, heart disease, chronic obstructive pulmonary disease (COPD), diabetes or cancer.

Work one-on-one with an advocate who may have tools and resources that can help you better understand your treatment options. If you require surgery, your advocate can provide you with information to help walk you through what you may expect during hospitalization and help you prepare for recovery.

When you join the Maternity support program, you’ll get confidential over-the-phone support from a nurse. And you’ll work one-on-one with that same nurse throughout your pregnancy — and up to six weeks after delivery.

Your maternity nurse can help with questions like:

• How much do my benefits cover?
• Are there ways I can save money?
• What can I expect before and after delivery?

If you or a loved one has been diagnosed with cancer, call for support—now and throughout the treatment journey. Joining the Cancer support program connects you one-on-one with a dedicated cancer nurse who is trained to help:

• Answer your questions.
• Talk about the emotional side of cancer.
• Prepare you for surgery or other treatments.
• Show you how to find a primary care provider that’s right for you.
• Provide you with information about your treatment options.
